From care to cash — without losing the clinical evidence

Most administrative pain in home care comes from broken links: a visit that no longer maps to an assessed need, minutes nobody can justify, an expired order discovered after submission, an insurer query answered from memory, or a payment that never gets matched back to the invoice line. CareOps gives you one traceable chain, so each step carries the evidence the next step needs.

  1. 1Care need — assessed with the patient, dated and signed
  2. 2Evidence — observations that support the stated need
  3. 3Intervention — linked to the need, with a measurable goal
  4. 4Time — planned and actual minutes, per service category
  5. 5Invoice — every line resolves to a retrievable record
  6. 6Payer — the correct payer path, documented not assumed
  7. 7Response — queries answered with dated, evidence-linked facts
  8. 8Payment & reconciliation — matched, with an open variance register

Statuses stay explicit throughout — coverage, decisions and payments remain UNKNOWN until confirmed in writing or received. Nothing is assumed, and no reimbursement, insurer acceptance or time saving is promised.

Why this is different

Generic document packs give you files to fill in. CareOps is built around traceability across the whole care-to-reimbursement workflow.

Generic templates

  • Store documents side by side
  • Each form stands alone
  • Time and frequency left unexplained
  • Insurer queries answered ad hoc
  • Billing checked only when something goes wrong
  • No record of what is still unresolved

CareOps

  • Structures a single evidence chain
  • Every intervention links to a need and a goal
  • Minutes carry a documented justification
  • A repeatable, evidence-linked response SOP
  • A pre-billing audit before submission
  • An explicit variance and status register

Who it is for

Written in English and kept country-neutral by default, so it works for care professionals in Switzerland, Germany, Austria, Italy and English-speaking Europe. Country-specific rules are kept in separate modules — Switzerland is included today.

  • Independent nurses and self-employed carers
  • Home-care operators and small agencies
  • Care coordinators managing visits and rotas
  • Small healthcare service businesses
  • Agencies just setting up operations
  • Solo practitioners needing a structured starter kit

What it helps you do

Standardise intake

Use a repeatable client onboarding flow so nothing important is missed at the first visit.

Document visits clearly

Care visit checklists and handover templates keep records consistent across shifts.

Align with payer expectations

Documentation structure designed around the evidence insurers and public payers typically expect.

Build compliant processes

Operational outlines for consent, privacy and risk management that you adapt to local law.

Run a small team

Starter policies and role definitions to help you hire, train and supervise confidently.

Reduce admin time

Ready-to-use templates mean less time writing documents and more time delivering care.

Switzerland Professional Pack

Included in the Starter Pack at no extra cost. A Switzerland-focused module that teaches the structure of a robust nursing care plan and of insurer-facing documentation, written in our own words and referencing public Swiss legal and official sources only.

  • A 13-step care-plan structure: assessment, care need, contributing factors, observable evidence, measurable goals, interventions, frequency and responsibility, rationale, monitoring, reassessment, outcome, escalation and traceability
  • Swiss documentation guide covering the distinction between assessment/advice/coordination, examination and treatment care, and basic care
  • Guidance on nurse-led needs assessment with the patient and family, medical orders where required, and authorized-provider records
  • A clear separation between reimbursable nursing care and non-covered domestic or support services
  • An insurer-facing checklist: care need, assessment-to-intervention link, frequency and time justification, measurable goals, change evidence, dates, signatures and traceability
  • A professional response template for a reduction or denial request, using documented patient-specific evidence only
  • A reassessment and review checklist, plus an evidence mapping matrix (CSV) with source, as-of date and review status
  • A country-module architecture so further country packs can be added later without mixing jurisdictions
  • A reimbursement operations system: intake, coverage and authorisation evidence, assessment, A/B/C classification, orders, care-plan linkage, visit and time capture, submission, status tracking, residual financing, payment and reconciliation — each with explicit control gates
  • A pre-billing audit checklist that flags missing or expired orders, intervention-to-need mismatches, unmeasurable goals, unjustified minutes, A/B/C or domestic-vs-nursing ambiguity, duplicate or overlapping visits, missing signatures and attachments, deadlines and invoice-to-record differences
  • A claim status register (CSV) tracing patient → episode → assessment → intervention → visit → invoice → payer → decision → residual financing → payment → reconciliation → variance
  • A step-by-step SOP for insurer queries, reduction requests and denials: triage, deadlines, evidence retrieval, gap check, structured response, attachments, follow-up, outcome and escalation

Important: this module is a workflow and documentation aid. It is not legal advice, not a billing engine, and not a guarantee of reimbursement or insurer acceptance. It contains no proprietary nursing classification content or taxonomies; if your service uses a standardized classification system, you must obtain and use your own lawful licensed access to it. Always verify the current version of any legal source before relying on it.

Built for country-specific rules

Care documentation and payer expectations differ by country, and often by region. CareOps is designed around one jurisdiction at a time rather than a vague European average.

  • Switzerland first: the current Professional Pack covers Swiss care-plan structure, insurer documentation and reimbursement operations.
  • Further country packs are planned and will be released individually — one jurisdiction at a time, never merged.
  • Only public and official sources are referenced; no third-party proprietary classification content is included.
  • Every module carries an as-of date and a review cycle, so you can see when its content was last checked.
  • Rules from one country are never used as a default or approximation for another; a missing rule stays UNKNOWN.

You must still verify the current version of any legal or payer source and adapt every document to your own canton, contract and professional requirements before use.

Frequently asked questions

How and when do I receive the files?

After Stripe confirms payment, return to CareOps and enter the email used at checkout. If a paid sales record exists, a short-lived private download link is generated for that email only.

What format are the files in?

Five CSV spreadsheets and sixteen Markdown text documents. They open in Excel, Numbers, Google Sheets, Word or any plain-text editor, and you can adapt them to your own branding, language and workflow.

Is this suitable for my country?

The templates are operational starting points. You must adapt every document to your local laws, payer rules and professional requirements before use.

Does it include standardized nursing classifications?

No. The toolkit contains no proprietary classification content or taxonomies. It teaches an original documentation structure with neutral fields where you enter terms from a classification system you are separately and lawfully licensed to use.

Do I get updates?

This is a one-time purchase of the current Starter Pack. Major updates may be offered separately in the future.

Can I share this with my team?

You may use the toolkit within your own organisation or sole practice. Resale, redistribution or public sharing is not permitted.

Who is this for — and who is it not for?

It is for independent nurses, home-care operators, care coordinators and small care businesses who need a documentation and operations structure they can adapt. It is not for anyone looking for clinical decision support, a billing or scheduling software product, a legal compliance certificate, or a ready-made country-specific policy set that can be used without review.

What language are the files in?

English. The files are plain CSV and Markdown, so you can translate or rewrite them into German, French, Italian or any other language you work in.

How do I access and edit the files?

The CSV files open in Excel, Numbers, LibreOffice or Google Sheets. The Markdown files open in any text editor and can be pasted into Word or Google Docs. No special software, licence or account is needed, and the files stay yours to keep.

What is your refund policy?

As stated in the Terms on this page, CareOps Toolkit is a digital product sold as-is and all purchases are final. If something is wrong with your delivery, reply to your Stripe purchase confirmation and we will sort it out.

Important disclaimer: CareOps Toolkit provides operational and documentation templates only.
